CalAIM Initiative - Enhanced Care Management (ECM)

What is enhanced care management?

Enhanced Care Management (ECM) is a free, statewide Medi-Cal benefit introduced under CalAIM (California Advancing and Innovating Medi-Cal). 

Think of it as a massive upgrade to how California supports its highest-need Medi-Cal members. Instead of forcing someone with complex medical or social challenges to navigate a maze of doctors, therapists, and social workers on their own, ECM gives them a single point of contact to handle everything.

How It Works: The Lead Care Manager

The core of ECM is the Lead Care Manager. This is a dedicated person who coordinates all aspects of a member’s life that impact their health. 

Crucially, this care is community-based and "high-touch." Care managers don't just sit in an office waiting for a phone call; they meet members wherever they are comfortable—whether that is on the street, in a homeless shelter, at a doctor's office, or in their own home.

The 7 Core Services Provided

  •  Outreach & Engagement: Actively finding and building trust with vulnerable members. 
  •  Comprehensive Assessment: Creating a personalized care plan that looks at both medical and personal lifestyle needs. 
  •  Enhanced Coordination: Ensuring doctors, mental health providers, dentists, and specialists are all communicating.  
  •  Health Promotion: Helping members better understand and manage their chronic conditions. 
  •  Transitional Care: Guiding members safely through major life shifts, like being discharged from a hospital or transitioning out of incarceration. 
  •  Member & Family Supports: Educating and supporting the member's family or support network. 
  •  Social Resource Referrals: Connecting members to community resources like housing, food banks, and transportation. 

Who Qualifies? (Populations of Focus)

ECM isn't for every Medi-Cal member; it is specifically reserved for individuals facing intense, overlapping medical and social challenges. Medi-Cal categorizes these as Populations of Focus, which include: 


  • Individuals Experiencing Homelessness: Adults, families, or unaccompanied youth who are unhoused or at high risk of losing housing. 
  • High Utilizers of Emergency Care: People who have frequent, avoidable emergency room visits or unplanned hospital stays. 
  • Those with Serious Mental Illness (SMI) or Substance Use Disorders (SUD): Adults and youth actively struggling with severe mental health issues or addiction. 
  • Justice-Involved Individuals: Adults and youth transitioning back into the community after being released from a correctional setting within the past year. 
  • At-Risk for Institutionalization: Adults living in the community who require skilled care but want to avoid being placed in a nursing home. 
  • Nursing Home Transitions: Residents currently in a nursing facility who are capable of safely moving back into independent community living. 
  • Vulnerable Children & Youth: Kids enrolled in California Children’s Services (CCS) or those involved with the foster care/child welfare system. 

The Big Picture

Ultimately, ECM recognizes that health isn't just about medicine. If someone doesn't have a safe place to sleep or a way to get to the grocery store, their physical health will suffer. ECM attempts to fix the root social issues alongside the medical ones.
